One Care is inviting expression of interest for six General Practice Multi-Neighbourhood Leads who will provide senior clinical leadership to support the strategic development and delivery of neighbourhood-level health and care across the six localities.

The role drives integration across primary care, community services, social care, and secondary care, with a focus on improving population health outcomes, reducing inequalities, and enabling proactive, evidence-based, community-led decision-making.

The postholder will champion patient and resident involvement, strengthen multi-sector collaboration, and provide expert clinical insight to inform service redesign and transformation.

This position is not offered on an employed basis. Applicants must be able to secure release from their GP practice for the required time commitment or be able to invoice One Care as an independent contractor.
