Role summary
To understand the cardiology landscape and to align general practice priorities and those of the system, through the implementation of transformational projects which improve and develop cardiology pathways for patients.
Key duties
- Enable general practice to feed into the design, development, and mobilisation stages of a model of care that delivers the outcomes of the cardiology workstream objectives, reduces inequalities across the system and responds to local needs
- Develop and implement a general practice programme plan of cardiology priorities
- Develop links between secondary care and the community including general practice in improving cardiology services.
- Work alongside the single managed services team, which is working across the acute trusts as part of the acute provider collaborative
- Support the GP Collaborative Board (GPCB) to ensure sufficient resource available to general practice to deliver the programme plan
Salary and benefits
£46,148 to £52,809 depending on experience
